Size the manual as entries, not as a document

Approve each entry on its own, and keep the approval

A manual approved as a whole is approved by nobody in particular. Each policy and each procedure has an owner who signed the current version off on a date, and the software keeps that signature with the version. When the auditor asks who approved the expense policy, the answer is on the record, not in an email thread.

Review by the calendar, entry by entry

Every entry carries its own review cycle, and the software raises the reminder when it falls due. A twelve-month cycle across sixty entries is five reviews a month, which is a habit; the same sixty entries reviewed together every three years is a project nobody starts.
